March 2017 - NHS England have introduced changes to the home oxygen forms. Here is a summary of the changes:

- new home oxygen consent form (HOCF) combined with a new Initial Oxygen Risk Mitigation form (IHORM)

- updated HOOF. Now requires a declaration that both the HOCF and the IHORM have been completed

- the clinical code on the HOOF is now mandatory

- From 1st Aug 2017 - current HOCF no longer valid
